OFFICIAL-SENSITIVE ONCE COMPLETE
Please complete this form if you wish to ask us to delete your personal data (an erasure request). You do not have to use this form, but it will help us to deal with your request as quickly and effectively as possible if you do.
You can also use this form if you are making an erasure request on behalf of someone else. In that case, we will need you to confirm you have that person's authority to ask for deletion of their data.
Please note your right to have your personal data erased only applies in limited circumstances, ie:
- we no longer need your personal data for the purpose for which we originally collected/processed it;
- we are relying on your consent as the lawful basis for processing your data, and you are withdrawing your consent;
- we have a legitimate reason for processing your data, but you object to our processing and our reason does not override your interests;
- we are processing the personal data for direct marketing purposes and you object to that processing;
- your personal data was unlawfully processed;
- we must erase your personal data to comply with a legal obligation; or
- we processed the data to offer certain digital services to a child.
Even in the circumstances listed above, we may not be required to erase your personal data if we need to process it, eg:
- to comply with a legal obligation;
- for certain public interest reasons or for exercising the right of freedom of expression and information;
- to establish, exercise or defend a legal claim.
If you have any questions about this form or your request, please contact dpo@conviction.org.uk to discuss it further.
